Gynecomastia, casually known as “man boobs”, is a typical condition in men and boys. In this condition breasts expands and become bigger in size. This is mainly seen in infants, young men during puberty and in older age. This word is derived from Greek word Gyne means woman and mastos means breast.
For most men Gynecomastia can be a major issue as it can cause lot of social embracement and humiliation. Men feel humiliated because of their appearance of female like breasts. Boys or men affected with Gynecomastia tries to social gathering, events and taking showers in pools. What is the cause of gynecomastia in men ?
Hormone imbalance between the sex hormones estrogen and testosterone is the main cause of Gynecomastia. As estrogen (female hormone) hormone cause development of breast tissue while testosterone (male hormone) suppress the growth of breast tissue. When the level of estrogen hormone increases growth of breast tissue occurs in males. Obesity, steroids, certain medicines, hyperthyroidism are some other causes of Gynecomastia.
What are the components of Gynecomastia?
The Gynecomastia or man boobs consists of two main components
- Glandular tissue – which is firm and dense.
- Fatty tissue – which is soft.
The ratio of glandular to fatty tissue varies from patient to patient. In individuals with Gynecomastia there may be two types of tissues.
What are types of Gynecomastia?
Gynecomastia is usually of two types
- True Gynecomastia: Due to proliferation of ducts and periductal tissues.
- Pseudo Gynecomastia: Due to deposition of adipose tissue or to the presence of an excessive amount of skin.
What are the symptoms of Gynecomastia?

- The breast gland tissue swells and become tender
- Diameter of areola increases and chest becomes symmetrical
Treatment of gynecomastia
The gynecomastia treatment in male also known by various name Male boobs surgery, male boob reduction, male breast treatment. Gynecomastia surgery is done under local or general anaesthesia under the following steps
- In Gynecomastia surgery first marking is done from where all the abudance fat and glandular breast tissue are to be removed.
- After that anaesthesia is administered this can be either local or general. The choice of anaesthesia depends on some factors such as type and grade of Gynecomastia, obesity, underlying medical conditions.
- Very small incision is being made near nipple areolar region and VASER probe is being inserted for breaking fat and fibrous tissue. Fat and fibrous tissue are broken and by ultrasonic cavitations and a proper contour of the whole chest is given.
- When no fat is seen to be coming out from the area then suction is being stopped.
- The extra glandular breast tissue is being removed by enlarging incision. After the surgery only a small scar is visible which gradually goes away with time.
What are the precautions you have to take after Gynecomastia surgery?
After surgery, a small dressing is done over the chest and pressure garment is applied to minimize swelling and support your new chest contour as it heals. Some of the precautions which you have to take following your Gynecomastia surgery are
- You can take bath next day after the procedure and after that no dressing is applied. The sutures will be removed around 10 to 14 days.
- You will be prescribed 7 days course of antibiotics and analgesics after the procedure.
- You can resume normal activity once you are comfortable to do movements. Mostly 1 or 2 days rest is enough to recover.
- You can start exercise after 1 or 2 weeks once you are comfortable.
- You have to avoid driving for first 24 hrs and if possible accompany a person with you.
When you will see the final result of the procedure?
After the surgery there will be small scar which will be not at all visible. The scar will heal within 6 months and after that they will not at all visible. Within a month after surgery you will start seeing the change in shape and reduction in breast size.
What are the safety and risks of Gynecomastia surgery?
The procedure is completely safe and there are no chance of coming back again .The side effects after the gynecomastia procedure include
- Swelling
- Bruising at some part of the chest
- Pain and Discomfort
- Numbness
These are temporary and will resolve within 7 to 10 days after the procedure. To reduce these side effects you have to wear compression garments for first 2 months.

This depends on the type of work you do and the extent of the male breast reduction procedure. Usually it’s between 2 to 3 days, but healing time varies for each individual and should be discussed with your surgeon.

The treatment of gynecomastia is done under local anesthesia with some light sedation. After the procedure you will slightly drowsy for first 2 hrs then you can start orally first sips of water and then some liquids. You can have light food in the night.

No drain is kept for the gynecomastia surgical procedure.
Q. Is the surgery really that painful?
Judging pain following surgery is very difficult since everyone’s threshold for pain is different. Most patients comment that the gynecomastia surgery was nowhere near as painful or uncomfortable as they thought it would be. Most patients do not have severe pain following VASER liposuction and surgical excision. The pain is most often described as minor or moderate in nature and severity. Yes you’ll have a prescription for a pain medication strong enough to relieve your postoperative discomforts.
Q. What will my chest look like when the dressings are removed?

Q. If my nipples feel differently after the surgery, what can be done?
In most of the cases nipple sensation becomes normal, time may take more than few weeks to few months.
Q. How long does the surgery take?
Generally, the procedure usually takes one and a half to two hours and once you are comfortable then we will discharge on the same day.
Q. Do I have to stay in the hospital?
The male breast reduction is a day care procedure so no admission or stay is required in the gynecomastia clinic. We prefer to use local anaesthesia with sedation, in which case you’ll sleep through the entire operation. Within few hours of the surgery you can yourself go back to your home comfortably spend the night in your home.
Q. Will the gynecomastia come back?
The glandular component will not return unless you stimulate the tissue with steroids or other drugs, in which case it can become enlarged again. The surgery removes most, but not all, of the glandular tissue. We surgically excise the gland and fibrofatty tissue to achieve the best cosmetic result.
Q. Can liposuction be done alone?
Yes, liposuction alone can produce a good result, but only if you have pseudo gynecomastia. As mentioned earlier, liposuction does not remove glandular tissue and as such is not the most effective approach.
Q. Can I drive myself home?
No, you cannot drive yourself home after surgery. You will need assistance for the first 24 hours.
Q. Why do I have to wear a vest after surgery?
The vest creates compression for the “dead space” created by the removal of the glandular and fibrofatty tissue. The skin will adhere to the underlying tissue and the swelling will be greatly reduced. It is extremely important to follow instructions regarding wearing the vest after surgery.
Q. Do I need to lose weight before I have the procedure?
No, your weight may be a concern of yours, but it will not affect the surgery. If you have pseudo gynecomastia (“fatty tissue”), then this will naturally reduce with weight loss.
